Serial Claim Request Form
There are many reasons why some serial and book orders do not arrive within reasonable amounts of time.For most titles Millennium is set up to inform us when issues are late, but for some titles we may be unaware that there is a problem until we are informed by customers. The Serials Receiving Work Team seeks your help in identifying these problems. The following form may be used to claim serial issues that we are missing.
Monographic claims are done within the Ordering Work Team monographic claims form. If you have questions about the Library's information resource (book buying) priorities please contact the selector responsible for the appropriate subject area.
When completing the following form you may include as much or as little information as you have on the item. The more information you can include, the better. Please use the comment box in the following form to include any information that is not asked for in the rest of the form. If you can locate the order or bibliographic record number from the Millennium record, please include this in your claim request as this helps make our process more efficient.
