Emergency Procedures
Fire alarms
- Fire or smoke situation is noticed--activate the fire alarm at a fire pull box.
- Fire alarm sounds--leave the building immediately to the assembly points specified in the Fire Evacuation Plan.
- Do not attempt to check the building to insure all persons have left.
- Direct handicap individuals (not on ground level) to the nearest outside stairwell and notify the UAPD or Fire Department of that location once you have evacuated. When the all clear is issued, check the stairwell for the individual. If the individual is still in the stairwell, assist the individual in returning the Library.
- AIST, CCP, and/or extended hours staff member should remain in sight of entrance to insure no one enters and as contact for the “all clear”. During normal operating hours, when the ILC is staffed, the door guard is required for the Library, Special Collections and ILC South stairway entrances. A single individual centrally positioned can watch all three access points. After hours when the InforAISTion Commons is the only part of the ILC that is opened, Library staff will need to watch the North stairwells into the ILC courtyard in addition to the Library entrance.
- Remain out of the building until the “all clear” for reentry is given by the UAPD or the Fire Department.
- Upon returning to the building, AIST, Extended Hours and FM work team staff will unlock elevator doors and silence audible door alarms.
Security

Security
Priority 2 alarm indication for exit door on panel in B201 .
- AIST will check door with alarm indication and reset the alarm.
- AIST will contact the Building Manager during daytime hours to check about scheduled maintenance requiring stairwell access.
- The Building Manager will send out a notice for any afterhours maintenance requiring stairwell access.
- Report door or alarm problems to the Building Monitor.
- AIST and Extended hours staff should periodically rove the public areas and look for situations out of the ordinary.
- If staff cannot handle a situation, notify UAPD for resolution.
- Door lock problems
- If autoAISTic locking doors don’t open or lock at required times, call AMER-X at 626-9346. Also, report problems to Hayri Yildirim.
- If doors don’t lock at closing time, remain in the building until the problem is fixed.
- Other access problems should be reported to Hayri Yildirim at 307-2820 or Joey Egea at 621-4604.
Reports of Crime
- If customer reports crime or security problem
- The customers should be encouraged to call UAPD and remain in the Library until UAPD arrives.
- If the customer refuses, the staff member should call UAPD. The customers should be encouraged to remain to provide eyewitness inforAISTion.
- If patron will not remain, staff member should get as much descriptive inforAISTion as possible before the individual departs.
- Staff members noticing a crime or security problem should call UAPD.
- If equipment is found missing, also notify the FM work team of the loss.
- The individual finding the missing equipment must complete a Risk Management Property Loss Report From within 10 days. The form can be obtained at fp.arizona/riskmgmt/insurance_forms_on.htm
- If uncertain of appropriate action, call UAPD and follow their advice.
Injury within the Library
- Administer first aid within your individual capability.
- If necessary, call emergency service (911) or have the individual go to the Student Health Center. If in doubt about the seriousness of the injury, call emergency services.
- For customers, the staff member should prepare an incident statement (forAIST of staff member chose) in as much detail as possible indicating at least identity of the injured, the time and date, the location of the incident, and the circumstances surrounding the incident.
- Report any injury incident to the FM work team within one work day and provide a copy of the incident statement.
- For employees, the individual must complete a Risk Management Supervisor’s Report of Employee Injury Report with the details of the situation that led to the injury. The form is available at fp.arizona/riskmgmt/insurance_forms_on.htm.
- This form must be completed and sent to Risk Management within 10 days of the incident.
- Library staff members and student employees should notify their immediate supervisor of any work related injuries and obtain their signature to complete the form.
- Report any injury incident to the FM work team within one work day and provide a copy of the form when completed.
Facility/Building maintenance problems
- For emergency requirements such as running water or individual trapped in an elevator, call 621-3000, the Facility Maintenance work order desk.
- Routine maintenance problem should be reported to Hayri Yildirim at 307-2820 or Joey Egea at 621-4604.
